I hope this communication finds you well and that your child is excited about returning to our Kindy program (in their new and exciting location down in the Hall) on Tuesday morning.
As you may be aware, the Government of Western Australia requires all schools to hold records of their enrolled students immunisation history.
As such, with all our Kindy students now having turned 4, we require an updated Immunisation History Statement, which includes your child’s latest 4 year old immunisation details.
Please see the attached flyer below for details on how to download a copy of the Immunisation History Statement via the MyGov website.
If you haven’t already could you please forward a copy of this updated statement to admin@stdominics.wa.edu.au by Friday, 20th August (Week 5).
